What is CO2 Fractional Laser?

Fractional CO2 Laser is a new technology that corrects many dermatological and cosmetic skin conditions through one procedure. The procedure corrects conditions such as superficial lesions, scars, skin texture, wrinkles, As the laser scans the surface of the patient’s skin, it will create thousands of micro-thermal zones in the treated areas which will promote the production of new collagen, resulting in healthier, smoother and younger-looking skin.

How long does the CO2 Fractional Laser procedure take?

The actual CO2 Fractional Laser procedure doesn’t take more than about 20 minutes, but in total you should expect to spend about 1.5 hours in the treatment room in order for the therapist to add the topical numbing cream and for it to work.  After the laser session is over, you will be asked to stay for another 20 minutes to cool down the skin through a cooling treatment.

How is the CO2 Fractional laser procedure performed?

A fractional CO2 Laser is exactly what it sounds like – fractional. This means that the tiny but intense laser energy micro beams create areas of treated tissue surrounded by untreated, healthy tissue. Over the coming days, weeks and moths, the body’s own healing process will create new, healthy tissue to replace the areas of affected tissue – thus improving the skin condition being treated.

Through my line of work, I’ve tried similar procedures before, such as Fractional Radiofrequency and Ultherapy by Ulthera. When starting the session, I’m convinced that this will be just as painful as the treatments mentioned above. To my relief I’m happily surprised about the CO2 Fractional Laser not being at all as painful as expected. I can definitely feel the minor “buzz” when the laser is projected through my skin, however the laser feels more like a tingling sensation than as pain. The smell of burnt hair (when the laser burns the tiny facial hairs) is actually more apparent than any pain from the laser. A great help for this is undoubtedly the facial numbing cream applied to my face 30 minute prior to the session. Dr. Rujirat is also extremely gentle and careful, constantly asking if I feel all right or need a small break, which is not needed.

To cool my face, an assisting skin therapist is holding a cooling handle, blowing soothing cold air around my face. After 30 minutes, most of the pain is gone even though it still feels slightly warm, and after two hours, I don’t feel any pain at all.

A fractional laser skin treatment is far more comfortable than traditional laser treatments and you will most likely feel like you’ve gotten a bad sunburn, which will pass within the first couple of hours.

CO2 Fractional Laser Results

The results from a Fractional CO2 Laser will be visible after about one week, once the treated area of the skin (little brown scabs) will peel off. For me, this took about one week. After that, I was able to see my new, fresh skin underneath. Although seeing apparent differences after 1-2 weeks, the full results came about a month after the procedure. My skin got a new healthy glow, the overall skin texture was improved, my pores were diminished and some fine lines under the eyes decreased. Along with the above, the treatment also took years of sun damage off my face allowing me to go without makeup and look more youthful with an even skin tone.

How many sessions of CO2 Fractional Laser are needed to obtain results?

Once the outer skin has peeled off in about a week, you will be able to see the instant results. Even though one treatment is enough to view apparent results, you will benefit from having the treatment done once every month or once every two months (depending on your skin type and condition) for about three times. According to some dermatologists, the effects are thought to lasts up to 8-10 years.

Benefits of CO2 Fractional Laser

One of the great advantages with CO2 Fractional Laser is the fact that the downtime is minimal. Straight after surgery, the treated area will be swollen and red, but in comparison to similar fractional procedures, the downtime is significantly minimized due to the laser beam being microscopic.

Another benefit is the increased comfort using Fractional CO2 Laser. The fractional array of micro beams will not cause bulk damage to the skin as traditional ablative skin treatments will. Through this, the fractional CO2 Laser is much gentler and the only anesthesia you will need is through applying a topical numbing cream.

The third benefit of Fractional CO2 Laser has to do with the increased safety using this skin procedure. Following the procedure, each tiny fractional column leaving a tiny scar re-epithelializes within a day or two. This minimizes the risk of infection, scarring or any long-term pigmentation changes.

Side Effects of CO2 Fractional Laser

Common but minor side effects from a fractional laser resurfacing include swelling and redness, which lasts for about two days. In my case it lasted for about one day. When leaving Genesis Skin Klinik, the treatment area is very red and hot. The nurse informs me that I should expect to be red until the next morning and that I might need an ice pack to cool down the face when going to bed at night. However, in my case, the redness fades away after about four hours, and so does the pain.

As with any laser surgery or laser treatment, patients with darker complexions may be at risk for pigmentation loss.

A Fractional CO2 Laser procedure is most commonly used on the face, neck, chest, hands and arms however generally the greatest results will be seen in the face, as this is generally where we have the largest pores and prominent wrinkles.

Dermatology of Today – Interview with Dr. Pravit Asawanonda, Chulalongkorn University, Bangkok

During last weekend’s International Congress in Aesthetic Dermatology 2012 (ICAD 2012), held in Bangkok, Thailand, Novasans had the opportunity to sit down and talk to one of Thailand’s leading dermatologists, Dr. Pravit Asawanonda, M.D. at The Faculty of Medicine, Chulalongkorn University, Thailand and also presiding over this year’s scientific committee at ICAD. During our talk, Dr. Pravit shared his thoughts on current and future trends in dermatology, and why anti-aging and aesthetics have become such sought-after treatment areas in recent years.

You are working at the Dept. of Dermatology at Chulalongkorn University in Bangkok. Are you also practicing at other hospitals in Thailand or elsewhere?

Yes, I have been practicing at Bumrungrad International Hospital for a little more than ten years.

You have a degree as Doctor of Science from Boston University, along with a Fellowship in Phototherapy & Laser at Harvard Medical School, both in the States. In your eyes, how important has your international training been for you professionally?

I would say that it has been very important for me in my practice at an international hospital here in Bangkok. As about 50% of my patients come from overseas, they feel more comfortable knowing that I have been trained and educated abroad.

50% international patients is a high number. Do you know why there are so many international patients looking for dermatology treatments in Bangkok?

When a patient is traveling to Bumrungrad from overseas for a major medical procedure, the family usually accompanies the patient. Once the family members are here, they seek treatment for minor procedures within dermatology. Bumrungrad attracts a lot of international patients from the Middle East so the majority of the patients I see come from this region.

Dr. Pravit, in your role as presiding over this year’s scientific committee, you are one of the organizers behind ICAD 2012. In your opinion, what is the purpose behind this event?

This is the fourth ICAD congress held here in Thailand. During the first two congresses, there were two main focus areas that were not intermixed; aesthetic dermatology and anti-aging and hormone. After receiving suggestions from the participants on combining the two areas, we did for the third congress. This has been very successful as we generally get a better exposure on the various areas.

During this congress, one of the main focuses has been anti-aging medicine and hormone therapy. Could you tell our readers a little more about the developments in this field?

Although not trained in anti-aging, I believe that anti-aging and hormone therapy is a very important area that a lot of dermatologists were lacking knowledge about in the past. Anti-aging was developed and viewed as a separate discipline. This notion has changed in the last years as we are seeing a growing aging population and a larger overall interest in staying healthy and vital. As once by some looked upon with a frown, it is now accepted even among the mainstream dermatologists.

Thailand is viewed to be on the forefront for aesthetic dermatology. What are the reasons behind this expansion in interest among the Thai population in these kinds of treatments?

I think what we’re seeing is a global trend, both in the consumer and the practitioner section and interest. There are however certain countries where aesthetic dermatology is more common, such as Japan and South Korea.

What trends can you pinpoint in aesthetic dermatology right now?

At present we are seeing more and more fillers and injectables. Although we are also seeing developments in terms of technology for machines, they are associated with big investments, and can sometime become out of fashion quite fast. Injectables are less complicated to use in many cases and the result obtained is generally very good. Another trend is the comprehensive approach on treatments. Today, you don’t use one individual treatment or procedure but a combination of several to obtain the best result. The approaches can include diverse methods such as diet therapy, breathing exercise, detox and light therapy.

Which are the most popular aesthetic dermatology treatments amongst your patients?

I myself only perform laser treatments. But generally speaking, injectables are very popular. Before, many patients used to come to me for hair removal, however today they are more interested in treating pigmentation, veins and wrinkles.

What are the variations in interest for aesthetic dermatology treatments in Asia vs. the Western world?

The demand is very similar be it Asian, European or Middle Eastern patients. However, as the face and body features vary between the different groups the problem areas and treatment methodology might also differ. The expertise between the regions is the same, but in Asia, there is often a limitation in terms of products on the market. In Europe, there are probably 50-60 fillers on the market, yet in Asia the range is not as big.

On a personal as well as professional level, which of all the presentations during these intense days are you most looking forward to, and which was most interesting and groundbreaking?

As the conference has just started, I’m mainly looking forward to the stem cell presentations.

South Korea is serious about Medical Tourism

The government’s medical tourism program and brand campaign Medical Korea, established in 2009 to promote inbound medical and health care travel, is taking new measures to increase the influx of international patients to South Korea. The government has now adopted a registry system for Korean medical institutions and agencies where so far over 2,000 organizations are registered, along with issuing special medical treatment visas and operating an around-the-clock medical call center for patients to contact.

2011 Awards to South Korean Top Medical Tourism Service Providers

On Dec. 21, 2011, Medical Korea also held a ceremony awarding the top medical tourism service providers and medical facilities in South Korea catering to international patient. In total, 10 outstanding medical tourism service providers and two special contributors were awarded along with three medical tourism agencies being given certification. Some of the medical institutions that were awarded this prestigious award were Seoul National University Hospital, Seoul St. Mary’s Hospital, JK Plastic Surgery Center, Arumdaun Nara Clinic and Yonsei Severance Hospital.

More South Korean Hospitals are Obtaining International Accreditation

It’s not only the South Korean government who is stepping up to meet patient demands. A growing number of medical facilities have received or are aspiring to obtain the Joint Commission International Hospital Accreditation. A total of 22 South Korean hospitals have now been accredited, including Seoul St. Mary’s Hospital, Severance Hospital, Korea University Anam Hospital and Ewha Womans University Medical Center.

International Patients Are Choosing South Korea

According to the South Korean Ministry of Health and Welfare, 81,789 foreign patients visited Korea for some kind of medical treatment in 2010. This is an increase of 36% from 2009. The total revenue from treating international patients also nearly doubled to $88 million in 2010. Most patients coming to South Korea for treatment are still out-patients (64,777) and the most popular countries of origin are the US, China, Japan, Russia and Mongolia. 14% of these had skincare-related treatment and plastic surgery, while 13,5% received internal medical treatment. 43% sought procedures in local medical centers while 20,5 opted for general hospitals and 23,5% chose clinics.

Join us for TEMOS Conference on Monday

Next wee, the 21-22nd of November, it is time for the 3rd International TEMOS Conference, this time at the Merzedes-Benz Center in Cologne. In its role as a medical tourism guide, Novasans attends the conference as media partners. The Managing Director of Novasans, miss Victoria Svederberg will also be one of the speakers at the workshop Best Practice in Case Management: more than high level medicine. Vitalize your skin through Lumenis One and 360 Degree Cryo Therapy – results, benefits and potential side effects

Through her gentle care, I hope to boost my skin and facial complexion.

Combine your skin treatments for optimal results

After Dr. Rujirat Vongtongsri has examined my face and its condition through the Visia Skin Analysis Machine, we both have a clear understanding of the current state of my skin (and the to me appalling number of UV Spots), my brown spots (visible to the naked eye), rosacea (red areas) and wrinkles that I hope to eliminate. In order to treat the various conditions, Dr. Rujirat recommends a combination of Lumenis One IPL treatment and vitamin infusion through Cryo Therapy, which I eagerly try.

Intense Pulsed Light (IPL) is a non-invasive laser-like treatment that can correct a range of benign skin conditions, such as sun damaged skin, freckles, uneven skin tone, acne, rosacea, birth marks and age spots. Lumenis One is considered one of the safest, most effective and most comfortable IPL treatments on the market. It can also be used to minimize pore size, broken blood vessels, and to remove hair. In my case, Dr. Rujirat mainly uses Lumenis One to minimize the number of UV and brown spots in my face.

How is Lumenis One performed?

Lumenis IPL treatment is gentle, quick and non-invasive. The therapist starts by thoroughly cleansing my face, before carefully covering my eyebrows so for them to stay unaffected by the strong light. The eyes are also covered to stay out of the strong light. Once the preparation is completed, Dr. Rujirat carefully places the applicator on my face and gently releases the intense pulsed light, which is changed to heat energy once entered into the skin. This results in photothermolysis of the target chromophores. Through reaching into the deeper layers of the skin, it will also boost collagen production. The process usually takes between 20-30 minutes.

Does Lumenis One IPL hurt?

Lumenis One is not a hurtful procedure. You will feel a slight tingling sensation when the light is released into the layers of the skin. The feeling can be resembled to a slight buzz into the deeper tissue, and when you smell the scent of burned hair, it is actually the prevalent fine facial hair that is affected by the strong light. Dr. Rujirat is exceptionally careful, and asks me at least ten times during the short procedure if it hurts, which I decline every time. I also don’t feel any pain or discomfort after the procedure. I walked out of Genesis Skin Klinik feeling completely refreshed, satisfied, and happily surprised about the lack of any potential ache.

Are there any side effects to Lumenis One IPL?

Although close to non-existent, the few side effects that you might experience are redness or swelling after the IPL treatment. Very rarely, there may also be some bruising or blistering. The face might feel tight or warm just after the treatment, which should settle within the first hours. After having the Lumenis One IPL done by Dr. Rujirat at Genesis Klinik, I did not feel any discomfort, redness or swelling at all and walked straight back to the office without anyone noticing any difference to my face, apart from a colleague pointing out that my face looked radiant. Whether the immediate results came from Lumenis One or 360 Degree Cryo Therapy, I can’t tell, but no matter what it helped.

What kind of results can I expect from Lumenis One IPL?

After Lumenis One, you will feel rejuvenated while having a more even skin tone and a healthier radiance. The exact results will vary depending on the condition of your skin and the severity of your condition, but already from one session you will see an apparent result. Within a few days, several of the underlying sun spots came up to the surface of the skin making my skin tone more uneven than before. However Dr. Rujirat explained to me that this is a natural process occurring when the skin rejuvenates itself from inside and when the before hidden spots are removed from the skin layers, and scrapes off.  Once removed, my face has a completely new, healthy glow to it.

How many sessions of Lumenis One IPL is needed for results?

After only one treatment, you will notice an improvement on your skin tone. For optimal results, you may need between 4-6 treatment sessions depending on your skin’s condition. To allow my skin to recuperate between each IPL treatment, Dr. Rujirat recommends me to come back in one month for the second session.

Vitamin Infusion through 360 Degree Cryo Therapy to re-energize your skin

To boost my skin even further, Dr. Rujirat also suggests a Vitamin Infusion in the form of 360 Degree Cryo Therapy. This effective treatment is used for different skin conditions, such as skin pigmentation, melasma, wrinkles, scaring, and enhancing the skin’s natural glow. Through creating temporary micropores in the skin, this transdermal technology can transport drugs and compounds boosting anti-aging. 360 Degree Cryo therapy breaks through the resistant skin layers and transports the highly concentrated potent active ingredients (such as minerals, vitamins, enzymes, plant extracts and amino acids) into the connective tissue, stimulating collagen and elastin growth.
